  • Complete the Columbia Suicide Severity Rating Scale (C-SSRS)
  • Identify level of risk
  • Contact building administration if moderate or severe risk is determined
  • Determine school response according to risk level
  • Complete documentation form
Mental Health Support Team (MHST)Re-Entry
  • Remain connected with the family to discuss the plan for return to school
  • Complete safety Plan
  • Complete re-entry plan with scholar and building administration
  • Inform staff of safety plan
  • File C-SSRs and all other forms in Electronic Health Record
  • Assign a staff member to schedule check ins with the scholar
All StaffFollow-Up
  • Ensure follow-up of the safety plan
  • Ensure follow-up of the re-entry plan
  • Review and revise safety plan as needed
