The Grand Rapids Public Schools, in partnership with Corewell Health, will offer influenza (flu) vaccinations at various locations for all staff. Flu vaccination will be very important because it can help reduce the overall impact of respiratory illnesses on the population and thus lessen the resulting burden on the healthcare system.

Vaccine Consent forms are linked below and will be available at each location.  

You must bring these three (3) items with you in order to get the flu vaccination.

  1. Completed Vaccine Consent form

  2. Your medical insurance card 
  3. A photo ID

All GRPS health plans cover flu shots at no cost to the employee - with proof of coverage.  

If you are on other insurance, please confirm that they will cover the flu shot. Corewell Health does work with most insurance companies, with the exception of Medicaid and United Healthcare.

If you do not have insurance, the cost of the flu shot will be billed directly to you.
